Transition Support: Services include helping individuals currently residing in nursing homes, congregate care, mental health facilities, prisons, jails, or other institution move back into less restrictive and more independent settings in the community. 

Institutional Diversion: Preventing unnecessary or unwanted institutionalization by providing community-based resources before a placement occurs.

Transition Planning: Creating individualized plans that address housing, personal care assistance, adaptive equipment, and local support networks.

As a Center for Independent Living, AIN receives federal financial assistance from the Administration for Community Living (ACL), U.S. Department of Health and Human Services (HHS)
